RESUMO

BACKGROUND: Conversion therapy for initially unresectable hepatocellular carcinoma (iuHCC) using lenvatinib combined with transcatheter arterial chemoembolization (TACE) plus a PD-1 inhibitor (LTP) has achieved promising results. However, further comparative research is necessary to evaluate the effectiveness and safety of conversion surgery (CS) for iuHCC. METHODS: Data for 32 consecutive patients with iuHCC receiving CS and 419 consecutive patients with resectable HCC receiving initial surgery (IS) between November 2019 and September 2022 were collected retrospectively. After propensity score matching (PSM), 65 patients were selected. RESULTS: Before matching, the CS group had longer EFS (not reached vs. 12.9 months, P < 0.001) and similar OS (not reached vs. not reached, P = 0.510) compared with the IS group. Similar results for EFS (P = 0.001) and OS (P = 0.190) were obtained after matching. The multivariable Cox model (HR = 0.231, 95% CI: 0.105-0.504; P < 0.001) and subgroup analyses confirmed that CS could improve EFS. The CS group had significantly lower incidence of microvascular invasion (MVI) than the IS group (3.1% vs. 50.4%, P < 0.001). Moreover, the two groups had similar safety profiles. CONCLUSIONS: CS is effective and safe for patients with iuHCC receiving LTP. LTP has the potential to reduce risk factors for postoperative recurrence, especially MVI, which may influence surgical decision-making.

Body sensor networks (BSNs) are playing a crucial role in tackling arising challenges during the COVID-19 pandemic. This work contributes by modeling and analyzing the BSN reliability considering the effects of correlated functional dependence (FDEP) and random isolation time behavior. Particularly, the FDEP exists in BSNs where a relay is utilized to assist the communication between some biosensors and the sink device. When the relay malfunctions, the dependent biosensors may communicate directly with the sink for a limited, uncertain time. These biosensors then become isolated from the rest of the BSN when their remaining power depletes to the level insufficient to support the direct communication. Moreover, multiple biosensors sharing the same relay and a biosensor communicating with the sink via several alternative relays create correlations among different FDEP groups. In addition, the competition in the time domain exists between the local failure of the relay and the propagated failures of dependent biosensors. Both the correlation and competition complicate the reliability modeling and analysis of BSNs. This work proposes a combinatorial and analytical methodology to address both effects in the BSN reliability analysis. The proposed method is demonstrated using a detailed case study and verified using a continuous-time Markov chain method.

Recently, indoor thermal comfort has received more scholarly attention than ever due to the COVID-19 pandemic and global warming. However, most studies on indoor thermal comfort in China concentrated on urban buildings in the east and north. The indoor thermal comfort of rural dwellers in southwest China is insufficiently investigated. Hence, this study assesses residents' indoor thermal comfort in a rural dwelling in Linshui, obtains the thermal neutral temperature of the rural area, and analyzes the thermal adaptation behavior of rural dwellers. The results reveal that the thermal neutral temperature of rural dwellers is 29.33°C (operative temperature), higher than that presented in previous studies based on the same climate region. Indoor thermal conditions in rural dwellings are relatively harsh, but various thermal adaptation behavior of rural dwellers significantly improve their ability to withstand the harsh conditions. When people live in an environment with a (relatively) constant climate parameter (e.g., humidity), their perception of that parameter seems compromised. Most rural dwellers are unwilling to use cooling equipment with high energy consumption. Therefore, more passive cooling measures are recommended in the design and renovation of rural dwellings.

RESUMO

Psoriasis (PS) is a systemic, immune-mediated inflammatory disorder. However, the whole lymphocyte compartment and the potential pathologies of PS have not been fully characterized. In the present study, we examined whole lymphocyte subsets and signal transduction proteins using high-dimensional single-cell mass cytometry and a bioinformatics pipeline for an in-depth characterization of the immune cell subsets and protein profiles involved in pathways in the peripheral blood of patients with PS. We identified 15 major immune cell populations in T cell lineages and characterized various CD3+CD4+ Th and CD3+CD8+ T cytotoxic cell populations simultaneously across 24 leukocyte markers and 7 proteins related to the signal transduction pathways. High-dimensional analysis identified 3 new subsets that are abundant in PS peripheral blood, resembling CD3-CD4+ lymphoid tissue inducer cells, Tc17 cells, and CD8+CXCR3+ Tregs. We confirmed the CD3-CD4+ cells, and their features and functions, in an independent PS cohort. The use of single-cell mass cytometry allows systemic-level characterization of lymphocyte subpopulations and dysregulated signaling pathways in the blood of patients with PS, identifying abnormalities of different immune cell subsets. We validated that the CD3-CD4+ cells had elevated OX40 and decreased FRA2 expression, which were positively associated with the PS area and severity index.

Apple mealiness degree is an important factor for its internal quality. hyperspectral scattering, as a promising technique, was investigated for noninvasive measurement of apple mealiness. In the present paper, a locally linear embedding (LLE) coupled with support vector machine (SVM) was proposed to achieve classification because of large number of image data. LLE is a nonlinear lowering dimension method, which reveals the structure of the global nonlinearity by the local linear joint. This method can effectively calculate high-dimensional input data embedded in a low-dimensional space manifold. The dimension reduction of hyperspectral data was classified by SVM. Comparing the LLE-SVM classification method with the traditional SVM classification, the results indicated that the training accuracy obtained with the LLE-SVM was higher than that just with SVM; and the testing accuracy of the classifier changed a little before and after dimensionality reduction, and the range of fluctuation was less than 5%. It is expected that LLE-SVM method would provide an effective classification method for apple mealiness nondestructive detection using hyperspectral scattering image technique.

PURPOSE: To study the clinicopathological change on "Multiple Evanescent White-dot Syndrome (MEWDS)" through fundus angiography analyses. METHODS: Examining the case with fluorescein angiography (FFA) and indocyanine green angiography (ICGA), visual field and following up for 15 months. RESULTS: There exist multiple white dots in the fundus photo and angiography. The changes on ICGA maintain longer than fundus photo and FFA. Papillae may be edematous and macula could present with special hypofluorescent flecks. CONCLUSIONS: MEWDS has specific angiographic features. Its pathological changes involve deep retinal pigment epithelium (RPE) as well as choroidal capillaries. The prognosis of the visual acuity for MEWDS is very good.
